Required Forms
Prior to any camp-out or event, a B.S.A. Permission Slip must be obtained for each scout in attendance.  The Permission Slip can be downloaded at any time.

Each year scout needs to have a physical and have a completed B.S.A. Medical Form on file.  This form is required before attending summer camp (Long Term) Camping Weekend (Short Term) and any other Scouting Event.  The medical form can be downloaded at any time.
